To promote high quality clinical care, improve patient overall well-being, and strengthen HEDIS performance, Point32Health encourages our provider partners to schedule patients for annual preventive services and appropriate screenings. Proactively scheduling routine visits will help close gaps in care and maximize HEDIS outcomes.
Preventive services may include services such as cervical cancer screenings, depression screenings, diabetic eye exams, routine lab work or managing and controlling conditions such as hypertension and diabetes.
Point32Health routinely provides many practices with member-level gap in care quality reports for members of our Harvard Pilgrim Health Care Commercial plans, Tufts Health Plan Senior Products plans (Tufts Health Medicare Preferred and Tufts Health Plan Senior Care Options), and Tufts Health Public Plans (Tufts Health Together and Tufts Health RITogether). We recommend that providers take advantage of this valuable resource and suggest monthly monitoring of these reports to close gaps for patients who may be overdue for certain health care services, laboratory tests, or screenings.
For Tufts Health Plan Senior Products, gap in care reports are shared via Electronic Data Download (EDD) mailbox (for groups with established access), and/or sent via secure email when a provider group contact is established. For questions on Senior Product gap reports, please each out to: Star_Team@point32health.org. Harvard Pilgrim gap in care reports are available through HPHConnect and accessible through secure FTP. Gap in care reports for Public Plans are sent to provider group quality teams through secure FTP or email.
